Colonial Park Care Center is a comprehensive skilled nursing facility dedicated to providing high-quality whole-patient care. Through its comprehensive onsite services including cardiac, orthopedic, wound and memory support, our center has come to revolutionize senior care,, helping its clients maximize their health, well-being and overall quality of life during these critical elder years.

The center seeks a Social Services Director to oversee all social services, including the evaluation of individual resident’s social needs and devising comprehensive care plans accordingly. The Social Services Director will be a critical cross-departmental link at the facility, working with medical and non-medical personnel to support residents in a comprehensive way, and ensure top quality total-patient care.

Responsibilities

Responsibilities include but are not limited to:

• Managing the overall social services program for the Center, including internal matters, as well as community/civic health projects
• Developing and maintaining social services objectives and standards along with specific policies and procedures for the center
• Establishing and implementing a method of assessing the social and emotional needs of each resident, and ensuring the social services component of care plans are developed and implemented
• Providing alternatives to drug therapy and/or restraints by understanding/communicating reasons behind resident behavior, and developing compassionate plans to serve all parties’ needs
• Aiding in discharge services, including community congregate living outreach, intake services for those returning home and transitions to hospice care
• Making arrangements for adaptive equipment, personal items and clothing, as needed
• Maintaining contact with families and providing updates to residents’ condition and progress, with patient permission
• Assisting residents with legal and financial matters, such as applying for pension, lawyer referrals and aiding with funeral/preplanning arrangements
• Developing and executing in-service training for relevant personnel
• Providing counseling/support for residents’ families

The Social Services Director will be a critical cross-departmental link at Colonial Park, working with medical and non-medical personnel to support residents in a comprehensive way, and ensure top quality total-patient care. Applicants must understand the foundation of eldercare support, and display extreme empathy, compassion and patience at all times. The successful candidate will have some experience in geriatrics/gerontology, and have worked in a Long Term Care facility or in medical social work. A bachelor’s degree in social work or a related field is required.
