  • Developing and implementing treatment plans, and working in tandem with other onsite care providers to devise comprehensive, integrated wellness programs
  • Assessing client progress and setting success metrics, in partnership with integrated care teams
  • Implementing an ongoing evaluation/assessment system to ensure care plans are continuously refined and enhanced
  • Developing comprehensive in-home care plans based on clients’ specific needs, external environments and care givers, then training family/care providers, as needed
  • Supervising Physical Therapy Assistants, rehab aides and volunteers

Serving as a Clinical Instructor for PT and PTA students, as needed

Qualifications

The resident Physical Therapist is an important piece of Apex’s multidisciplinary approach and, as such, requires an experienced, innovative and compassionate professional to carry out diverse duties to the center’s aging population. Relevant experience as well as a current license are required. A driver’s license and reliable transportation are also required, as this role requires some local travel. Vaccination from COVID-19 is a condition for hiring. A request for exemption can be submitted and will be reviewed and approved on an individual basis.
