As a skilled nursing facility, wound and skin care are essential steps in ensuring residents are healthy, comfortable and functional. The Wound Care Nurse will manage wound/skin care personnel, and carry out a number of hands-on tasks for the facility.


The Wound Care Nurse will manage wound/skin care personnel, and carry out a number of hands-on tasks for the facility, including:


  • Visits each new admission and readmission for skin assessment and reviews admission/readmission orders.
  • Informs nursing unit of wound progress.
  • Reports changes in condition of resident to charge nurse of unit; takes corrective action.
  • Attends in-service programs and continuing education programs geared toward geriatric residents and residents of various ages and incorporates the new information into practice. Assists with staff inservices regarding wound care.
  • Informs and educates residents and/or families about plan of care and encourages resident/family participation in attaining treatment goals.
  • Participates in wound care referrals.
  • Provides Business Office with appropriate documentation of wound progress, supplies utilized, etc., as requested.
  • Observes Standard Precautions and isolation procedures as directed.
  • Maintains confidentiality of resident information at all times.
  • Administering treatments according to physicians’ orders/plans of care, as well as facility policies and procedures
  • Performing weekly measurements of all wounds, and documenting findings accurately
  • Assisting in all care planning related to skin/wound treatment, and planning/recommending appropriate interventions
  • Researching cause factors and making appropriate recommendations to other departments to improve consistent, cohesive quality of care surrounding pressure relief, positioning, nutrition, hydration and the use of splints
  • Responding to/resolving emergencies in the absence of an RN
  • Additional skin/wound care services, as dictated by care plans and resident needs
  • Performs other duties as assigned by supervisor.


The Wound Care Nurse must be able to analyze and assist with complex resident care, and should be comfortable investigating/resolving problems, initiating therapeutic approaches to wound care/prevention and make on-the-spot decisions to best support patient needs. All candidates must be licensed as an RN or LPN in Pennsylvania, with at least one year of experience in long term care, wound care treatment and prevention required.
