Overview

Job Location: Geographically Between Los Angeles and San Diego

 

Grane Rx, a leader in post-acute care pharmacy solutions and services, seeks a positive, proactive and professional Consultant Pharmacist to join its client-facing pharmacy services team. The Consultant Pharmacist will be responsible for a host of critical tasks, including promoting Grane Rx’s comprehensive solutions, and communicating customer needs to members of the pharmacy team to ensure the best possible service.

 

Responsibilities

This role will require significant hands-on work with providers and clinic team members at the PACE organization as well as with the Grane Rx staff, with day-to-day responsibilities including:

  • Providing clinical, consultant pharmacist services to assigned Grane Rx PACE customers, in accordance with all company policies, guidelines, and professional standards of practice.
  • Performing medication regimen reviews for all program participants with a strong clinical focus on geriatric medication issues with the goal of improving participant quality or life and reducing the risk of adverse events.
  • Educating PACE staff regarding medication related issues
  • Visiting each PACE organization on a scheduled basis to promote increased interaction, collaboration, and to foster improved clinical and customer service outcomes.
  • Responsible for maintaining optimal customer service levels through establishing working team relationships

Qualifications

  • Graduate of an ACPE accredited School of Pharmacy with a BS in Pharmacy or Doctor of Pharmacy degree required
  • Board Certified Geriatric Pharmacist (or equivalent) preferred but not required
  • Resides in the state of California
  • Registered pharmacist in the state of California with license in good standing
  • Three of more years of PACE clinical pharmacist experience
  • Extensive medication management experience
  • Excellent written and verbal communication skills
  • Strong data analytics skills
  • Ability to work with teams and be a team leader
  • Ability to work independently when necessary
  • Ability to prioritize assigned tasks
  • Attention to detail and completion of all assigned tasks
  • Ability to work in a fast paced, challenging, clinical and technical environment
  • Reliable and consistent in timeliness, attendance, and quality of work
  • Travel to customer locations to perform job functions, overnight travel when necessary
  • Ability to be mobile in performing job duties
