What it is like to work as a RN Case Manager
Integrated care leader Grane Hospice Care, Inc. is expanding its core interdisciplinary teams, and seeks a compassionate, knowledgeable and hand-on RN Case Manager to join to its skilled clinical department. This nursing professional will administer care to patients in accordance with approved care plans, and work in conjunction with physicians, caregivers, families and other therapeutic personnel to ensure optimal comfort and the utmost in quality treatments, support and healthcare services.
Grane Hospice provides a higher level of hospice care through a low patient to nurse ratio, allowing for more care time whenever needed. This is a highly fulfilling role for the right candidate, and will enable the RN to work within an interdisciplinary team to deliver meaningful, critical support to a diverse population of patients and families. The RN Case Manager is expected to have excellent observation skills, good nursing judgment and topnotch written and oral communication abilities. This role requires a strong self-starter with the ability to thrive in both a team and independent environment. All candidates are expected to be graduates of accredited schools of professional nursing, and have current RN licenses through the State Board of Nursing. One year of RN experience is required, with specific hospice, medical, surgical or critical care work preferred.
Since travel is required, a valid driver’s license and reliable mode of transportation is also mandatory.
