LPN (Licensed Practical Nurse) with supervisory experience sought for CLINICAL CARE DIRECTOR role at Amber Hills Assisted Living Facility


Full Time – Mon.-Fri., 8 AM - 4:30 PM - Ebensburg, PA


Amber Hills Assisted Living Facility at Cambria Care Center offers 30 well-appointed assisted living apartments with a variety of comfortable layouts offering an emergency response system color TV with cable, wall to wall carpeting, and more! Amber Hills also offers a secure center for the memory impaired with 29 personal care studios. The environment is secure, self-contained and comfortable. We are currently seeking an experienced LPN (Licensed Practical Nurse) with strong supervisory/managerial experience for a multi-faceted Clinical Care Director role. This unique position provides staff and management support to the entire assisted living program as well as supervises nursing staff to ensure the utmost in quality care and service are provided at all times. This fast-paced role requires a compassionate, strategic critical thinker with strong nursing and managerial experience, as well as a desire to provide meaningful service to residents and families.


Day-to-day responsibilities of the LPN (Licensed Practical Nurse) Clinical Care Coordinator include:

  • Providing ongoing staff and management support across all areas of the assisted living program
  • Coordinating and supervising nursing staff and ensuring high standards of care for all patients and families
  • Develop, implement and update appropriate clinical standards of care as well as policies/procedures to ensure proper compliance
  • Instructing clinicians to ensure delivery of high quality care services, based on initial evaluations and assessments
  • Assisting clinicians in establishing immediate and long-term therapeutic goals, priority setting and development of care plans
  • Ensuring proper clinical documentation in all patient files, and reviewing records on a routine basis to identify potential action areas
  • Coordinating interdisciplinary team meetings with personnel to best facilitate care coordination
  • Assisting in screening/interviewing new employees, and aiding in new hire training
  • Participating in the quality management program
  • Maximizing patient and family satisfaction through ongoing review, evaluation and support services
  • Staying up date and knowledgeable of Bureau of Human Services Licensing Assisted Living regulations


Successful candidates will be a licensed LPN (Licensed Practical Nurse) in the state of PA; with at least 1-3 years of management/supervisory experience and successful work in a long-term care facility or an assisted living setting. All applicants must be able to work in interdisciplinary and self-directed environments and have good working knowledge of pain/symptom management as well as strong observation, top-notch judgment and excellent communication skills. All candidates must have a reliable mode of transportation. EOE.
