RN Needed to Join Fast-Growing Clinical Staff at Integrated Hospice Company


Full Time - Centre County


Integrated care leader Grane Hospice Care, Inc. is expanding its core interdisciplinary teams, and seeks a Registered Nurse to join its clinical ranks. The RN will be a critical member of this team, providing skilled nursing care and support from admission through discharge. Because of the nature of hospice work, the RN must exhibit extreme compassion, empathy and flexibility, towards patients as well as their families and caregivers.


Grane Hospice provides a higher level of hospice care through a low patient to nurse ratio, allowing for more care time whenever needed. This is a highly fulfilling role for the right candidate, and will enable the RN to work within an interdisciplinary team to deliver meaningful, critical support to a diverse population of patients and families. In this integrated care role the RN will manage a variety of key tasks and responsibilities including:

  • Providing professional nursing care by utilizing all elements of the nursing process
  • Assessing and evaluating patients’ statuses and providing care plan revisions, as needed
  • Initiating and sustaining orders for medications and treatments as prescribed by the physician and/or the approved care plan
  • Initiating and applying appropriate preventative and therapeutic nursing procedures and techniques, and communicating these applications to team members
  • Completing, maintaining and submitting clinical notes and records for all patients
  • Observing patient conditions and reporting changes to physicians and relevant team members
  • Assisting patients with daily living activities to facilitate self-sufficiency and comfort
  • Assisting patients, families and caregivers in providing strong continuity of care
  • Working with interdisciplinary teams to meet the emotional needs of patients, families and caregivers
  • Supervising ancillary personnel, as requested
  • Additional clinical and support services as requested by Clinical Coordinator/Director of Clinical Operations, or as needed by patients


All applicants must be graduates of accredited schools of professional nursing, with a current license to practice as an RN in Pennsylvania. Candidates should have at least one year of RN experience, ideally in a hospice, medical, surgical or critical care capacity. A strong understanding of hospice philosophy and principles of death/dying are important to this role, and to properly supporting both patients and families while at Grane Hospice.
