Integrated care leader Grane Hospice Care, Inc. is expanding its core interdisciplinary teams, and seeks a Nurse Practitioner to join its ranks. This is an essential role within the clinical department, performing a variety of medical services and working as a core member of the interdisciplinary team to maximize patient care, comfort and overall satisfaction. This senior-level role reports to the center’s Hospice Director.


Grane Hospice provides a higher level of hospice care through a low patient to nurse ratio, allowing for more care time whenever needed. The Nurse Practitioner will be responsible for a host of clinical and supportive tasks throughout Grane Hospice, including:

  • Performing clinical care include bedside evaluations, histories, physical examinations, radiological/laboratory interpretations, inpatient care follow up and general case management
  • Performing a variety of therapeutic measures, as needed
  • Working with physicians and other care providing to optimize patient care, and provides written reports to primary medical staff, as requested
  • Observing and reporting significant changes in patients’ conditions, then consulting with clinical staff, as needed
  • Aiding in the coordination of admissions and discharges, on select patients
  • Prescribing medication to the extent permitted by the Nurse Practitioner’s license
  • Ordering durable medical equipment for patients, as needed
  • Developing clinical documentation protocols and preparing/maintaining accurate patient records, charts and documents to support sound hospice practice and reimbursement for service providers


Because of the nature of hospice/home health care work, extreme compassion, empathy, flexibility and patience must be exhibited at all times. Applicants should have an appropriate master’s degree, plus be actively licensed to practice as a nurse practitioner in Pennsylvania. All candidates must have at least one year of practical experience with a strong working knowledge of hospice philosophy, medications and sterile techniques. Some travel is required for this position, so a valid driver’s license and reliable mode of transportation is also essential.
