    Integrated care leader Grane Hospice Care, Inc. is expanding its core interdisciplinary teams, and seeks an experienced, compassionate and dedicated Bereavement Counselor to oversee the organization’s bereavement program. This is an extremely critical role within Grane, providing vital counseling, support and other personalized grief services to patients, families and caregivers dealing with death and dying.

This is an extremely critical role within Grane, providing vital counseling, support and other personalized grief services to patients, families and caregivers dealing with death and dying.

     

    Grane Hospice provides a higher level of hospice care through a low patient to nurse ratio, allowing for more care time whenever needed.     Responsibilities

    Due to the nature of this role, extreme empathy, patience and flexibility are essential, as are strong communication and interpersonal skills. Responsibilities include:

    • Planning, managing and maintaining the Grane bereavement program, under the supervision of the Director of Chaplain & Bereavement Services
    • Coordinating the assessment and delivery of grief counseling for patients, families and caregivers, including one-on-one services, group sessions and follow ups
    • Overseeing strategic planning for the growth and further development of the bereavement program, including ongoing expansion, refinement and additional support services
    • Providing direct bereavement support (e.g. one-on-one counseling, group sessions, outside referrals) to hospice personnel coping with work-related grief
    • Providing bereavement information and referral services to callers from the community, and expand/maintain the community resource list, as needed
    • Participating in relevant activities such as in-service education, personnel meetings and committees, as requested

    Qualifications

    Ideal candidates will have experience working with patients, families and caregivers on death/dying issues, and will have a strong working knowledge of and commitment to hospice philosophy. An understanding of grief response and experience counseling individuals privately and in groups is also essential to success in this position.

     

    This role requires some local travel to visit patients and families in their home environments and, as such, a valid driver’s license and reliable mode of transportation is required.
