Territory: Mostly Dauphin/Cumberland Counties - occasionally York if needed
Hourly Pay Rate + Paid Travel Time + Mileage reimbursement
Integrated care leader Grane Home Health is expanding its core interdisciplinary teams, and seeks an experienced, compassionate Licensed Practical Nurse to join its clinical personnel in delivering quality health/social services to the center’s diverse patient base. This role requires significant hands-on work with an elderly and terminal population, so the utmost empathy, flexibility and dedication are essential.

Under the supervision of the RN Case Manager, the LPN will provide high quality nursing care to in-center and in-home patients, as prescribed by the physician and approved care plan. Because of the nature of Grane Home Health, the LPN’s responsibilities will be diverse and change based on the overarching needs of the organization and of the individual patients. All candidates must be graduates of accredited practical nursing programs, with current LPN licenses to practice in Pennsylvania. Additionally, candidates should have three or more years of nursing experience, with community health, hospice, medical and/or surgical work preferred. Candidates must be CPR certified.
Since some travel to patients’ homes is required, a valid driver’s license and reliable mode of transportation is also mandatory.
