    Integrated care leader Grane Hospice Care, Inc. is expanding its core interdisciplinary teams, and seeks an experienced RN with strong supervisory/managerial experience for a multi-faceted Clinical Coordinator role. This unique position provides staff and management support to the entire hospice program as well as supervises nursing staff and ensures the utmost in quality care and service are provided at all times.

     

    Grane Hospice provides a higher level of hospice care through a low patient to nurse ratio, allowing for more care time whenever needed.     Responsibilities

    This fast-paced role requires a compassionate, strategic critical thinker with strong nursing and managerial experience, as well as a desire to provide meaningful service to hospice patients and families. Day-to-day responsibilities include:  

    • Providing ongoing staff and management support across all areas of the hospice program
    • Coordinating and supervising nursing staff and ensuring high standards of care for all patients and families
    • Ensuring 24-hour availability of nursing staff to patients and families through approved on-call and staff schedules
    • Assisting the Director of Clinical Operations to develop, implement and update appropriate clinical standards of care as well as policies/procedures to ensure proper compliance
    • Reviewing, evaluating and supervising each case through home visits, conferences, record review and clinician service observation
    • Instructing clinicians to ensure delivery of high quality home care services, based on initial evaluations and assessments
    • Assisting clinicians in establishing immediate and long-term therapeutic goals, priority setting and development of care plans 
    • Ensuring proper clinical documentation in all patient files, and reviewing records on a routine basis to identify potential action areas
    • Coordinating interdisciplinary team meetings with hospice personnel to best facilitate care coordination
    • Assisting in screening/interviewing new employees, and aiding in new hire training
    • Participating in the quality management program, in conjunction with the Director of Clinical Operations and QAPI Manager
    • Maximizing patient and family satisfaction through ongoing review, evaluation and support services

    Qualifications

    Successful candidates will be licensed RNs with at least 3-5 management/supervisory experience and successful work in a hospice/home care setting. All applicants must be able to work in interdisciplinary and self-directed environments and have good working knowledge of pain/symptom management as well as strong observation, topnotch judgment and excellent communication skills. Because travel is required, all candidates must have a valid driver’s license and reliable mode of transportation.
