    Clinical Support Coordinator

    Integrated care leader Grane Hospice Care, Inc. is expanding its core interdisciplinary teams, and seeks a Clinical Support Coordinator. The Clinical Support Coordinator will provides administrative support to Regional QAPI Director for all quality, education, policy and procedure and process activities that impact all three home health offices.

    Responsibilities

    Responsibilities include:

    • Scheduling QAPI meetings for each home health office
    • Prepares QAPI materials in advance of meeting and distributes materials electronically
    • Maintains QAPI Binder for regional meetings including program overview, audit tool samples, minutes, etc
    • Attends and takes minutes for regional QAPI meetings and distributes minutes to QAPI team members
    • Assists with audits and data collection as directed by or in collaboration with the Regional Director
    • Follows up on and provides status reports of all QAPI activities and PIPs as directed by or in collaboration with Regional Director
    • Updates Admission Book and SOC packets to reflect current regulations and policies as necessary
    • Maintains updates to P&P, formats as necessary and distributes to Administrators, Clinical Managers
    • Prepares regional education calendar annually in collaboration with Regional Director
    • Prepares regional education materials including updated P&P related to clinical home health services and processes as necessary and distributes and tracks accordingly

    Qualifications

    The ideal applicant will possess the following:

    • Associate's Degree in relative field
    • 1-2 years of work related experience
    • Complies with and understands the hospice philosophy, protocols, and mission
    • Possesses excellent organizational, computer, and communication skills; must be proficient in MS Office Suite
    • Is flexible and self-motivated; ability to work with minimal supervision required
    • Is able to work successfully as a member of an interdisciplinary team
