Senior LIFE is an innovative home and community based Medicare and Medicaid funded program which provides all-inclusive healthcare services and support to seniors living in the community. At the center of this integrated approach is comprehensive support of members’ emotional and social well-being, with the LIFE Center Social Worker overseeing the successful development and implementation of many of these integral tasks.
The Social Worker will be a key participant in member interdisciplinary teams, weighing in on a host of medical, physical, social and emotional assessments and Plans of Care. Additionally, this professional will oversee a number of core programming and therapeutic responsibilities, including:
Due to the hands-on nature of this role, the ability to develop and foster positive, caring, productive relationships with elderly members and their families is essential, as is the desire to work in a unique environment with a unique, integrated approach to care services. Social Workers will work in-tandem with a variety of other internal and external care providers to ensure the best possible care and quality of life for members and, as such, increased sensitivities, empathy and discretion is required. Applicants must have superior communication and analytical thinking skills, with the ability to efficiently and effectively troubleshoot as needs arise.
A Master's Degree from an accredited School of Social Work is required. Candidates must also have one year of experience working with the frail or elderly population within a healthcare setting. Experience with the Medicaid application process a plus!
