Skilled SOCIAL WORKER Needed for Integrated Nursing Facility

 

Full Time - Monroeville, Pa

 

Woodhaven Care Center is a comprehensive skilled nursing facility dedicated to providing high-quality whole-patient care. Through its comprehensive onsite services including cardiac, orthopedic, wound and memory support, Woodhaven has come to revolutionize senior care in Allegheny County, helping its clients maximize their health, well-being and overall quality of life during these critical elder years.

To aid in this mission, the center seeks a Social Worker to assist the Director of Social Services in all day-to-day functions of the unit. This important role will oversee a number of vital services for our center, including the evaluation of individual resident’s social needs and devising comprehensive care plans accordingly.

Responsibilities

Responsibilities include:

  • Assisting the Director of Social Services in managing the fast-paced unit, including internal and external/community affairs  
  • Developing and maintaining social services objectives and standards along with specific policies and procedures for the center
  • Aiding in the establishment/implementation of a method for assessing the social and emotional needs of each resident
  • Providing alternatives to drug therapy and/or restraints by understanding/communicating reasons behind resident behavior, and developing compassionate plans to serve all parties’ needs
  • Aiding in discharge services, including community living outreach, intake services for those returning home and transitions to hospice care
  • Making arrangements for adaptive equipment, personal items and clothing, as needed
  • Maintaining contact with families and providing updates to residents’ condition and progress, with patient permission
  • Assisting residents with legal and financial matters, such as applying for pension, lawyer referrals and aiding with funeral/preplanning arrangements
  • Developing and executing in-service training for relevant personnel
  • Providing counseling/support for residents’ families

  • Additional responsibilities as assigned by the Director of Social Services

Qualifications

 

The Social Worker will also be responsible for stepping in during periods of his/her director’s absence and, as such, strong administrative, supervisorial experience, patience and outstanding interpersonal skills are essential. Candidates must have a bachelor’s degree in social work, with working knowledge of federal, state and Medicaid/Medicare regulations. Experience in a healthcare or medical social work environment catering to geriatric/frail clientele is preferred.
