Due to the nature of this work as well as the agency’s aging clientele, the successful candidate must be caring, patient and flexible, with a proven track record of excellent patient outcomes as well as strong communication and time management skills. The RN will be expected to be a motivated self-starter able to conduct in-depth clinical work with little or no immediate supervision.


Day-to-day responsibilities include:

  • Working with elderly in-home patients within an assigned geographical territory, creating personalized home health plans that comply with federal/state regulations, reimbursement guidelines as well as Grane policies and procedures
  • Performing initial and ongoing assessments of patient needs, including Outcome and Assessment Information Sets (OASIS) at appropriate points in time
  • Initiating plans of care, regularly reevaluating patients’ skilled needs and revising recommendations, as needed
  • Implementing preventative and rehabilitative nursing procedures
  • Reporting changes in patient condition to physicians and other relevant staff, and assessing the effectiveness of existing care plans
  • Supervising, directing and assigning routine nursing visits to an LPN, in keeping with his/her scope of practice and agency policies
  • Planning discharge services in conjunction with medical personnel, families and caregivers
  • Giving skilled care and treatments to patients who require intermittent nursing services, as needed
  • Obtaining necessary supplies and equipment to provide the most effective care
  • Providing referrals, as needed


All applicants must be graduates of accredited schools of professional nursing, with a current license to practice as an RN in Pennsylvania. Candidates should have at least three years of RN experience, ideally in a home health care, hospice, surgical or acute care capacity. 


Previous Psychiatric Nursing experience, minimally 1 year, is required
