LaurelWood Care Center is a 120-bed Skilled Nursing Center which includes a 20-bed Specialty Care Program offering Cardiac, Orthopedic, and Wound Care Program.


LaurelWood sits in a wooded, hilltop community in southern Cambria County. Beautifully landscaped, the grounds include a patio, walking paths, gardens, and shaded lawns that let you enjoy watching deer and other wildlife. Conveniently located off Route 271 (Menoher Blvd.), the Center is quickly accessible from virtually anywhere in Johnstown, so that friends and family can easily visit.


We retain a caring, accurate, professional and experienced staff. Our people take the time to show they care. Several physicians comprise our medical staff, including general practitioners and specialists.


To support the center’s ongoing success, LaurelWood seeks an LPN Case Manager to oversee the flow of inbound prospects and referrals, through targeted marketing, partnership development and community outreach.


Responsibilities include:

  • Developing comprehensive marketing initiatives designed to drive increased referrals and inbound leads, from partner care providers/services and individuals
  • Establishing partnerships with third-parties (physicians, care centers, etc) to best fuel the referral pipeline, and drive new prospects to LaurelWood
  • Prescreening candidates for admission, and coordinating relevant nursing assessments
  • Assuring the appropriate placement of new residents within LaurelWood upon admission
  • Submitting all insurance, financial and demographic information to the business office prior to new resident arrival
  • Functioning as a LaurelWood representative for families, insurances, physicians and other care professionals, as it relates to resident care
  • Overseeing the admissions staff, including an Admission Clerk and Admission Medicaid Clerk


Candidates must be licensed LPNs with a valid Pennsylvania License, and a strong working knowledge of federal/state regulations, Medicare/Medicaid and general insurance best practices. To be considered, applicants must have at least one year of experience in a skilled nursing facility or related healthcare environment with specific case management work.  Candidates must also be familiar with PCC, MDS, Care Plans, Insurance Reviews, D/C Planning and Nursing/Therapy Review.
