Weekday On Call for the Altoona Area

 

 

Integrated care leader Grane Hospice Care, Inc. is expanding its core interdisciplinary teams, and seeks a licensed, professional On-Call Nurse to provide in-home services, interventions and supportive care for weekends, holidays, evenings and overnight on-call duties. Due to the nature of this position, the On-Call Nurse must be flexible, exhibit a keen ability to work independently, execute sound judgment and successfully troubleshoot for a host of medical and non-medical situations.

 

Grane Hospice provides a higher level of hospice care through a low patient to nurse ratio, allowing for more care time whenever needed. Responsibilities

Tasks will vary based on the patients’ specific needs, with ongoing responsibilities including:

  • Assuming responsibility for evening, weekend and holiday on-call duties, as schedule, in keeping with hospice policies and procedures
  • Assuming primary responsibility for total care census, including assessing, planning, implementing and evaluating all phases of the nursing process
  • Communicating with attending physicians and other community resources to report changes and coordinate optimal care/use of resources for patients and families
  • Communicating with the Clinical Coordinator to ensure appropriate dissemination of information and follow through on the part of families and caregivers
  • Attending weekly interdisciplinary team meetings to maintain up-to-date patient records, troubleshoot problems, sets plans and action goals and work in tandem to adjust care plans, as needed
  • Obtaining data on physical, psychological, social and spiritual factors that may influence patient/family health status, and incorporating those findings into care plans
  • Monitoring physical care of patients and educating family, caregivers and volunteers in proper, approved techniques
  • Responding to medical or emotional crises and/or patient deaths, and make home visits to provide direct care, as appropriate
  • Providing appropriate counseling at patients’ times of death, and lending immediate support to families and caregivers
  • Carrying out additional assignments, as instructed

Qualifications

Candidates must have a current RN license to practice in Pennsylvania, with at least one year of varied nursing experience in an acute, oncology, home health and/or hospice setting. Applicants must be able to demonstrate a strong ability to quickly assess patient situations, and respond to emergent needs effectively and professionally. This role requires weekend, evening and holiday work, so scheduling flexibility as well as a valid driver’s license and reliable mode of transportation are required.
