Overview

RESIDENT CARE COORDINATOR needed for innovative Personal Care Facility

Full Time –  Monday Thru Friday 9am Till 5:30pm   

Amber House is a licensed Personal Care Center, which includes 49 rooms including 6 suites, and a Senior Wellness Program that encourages each individual to reach and maintain his or her highest level of functioning.

Additionally, extended care and overnight stays can be arranged through the Personal Care Respite Stay Program.

Restorative programs are a key piece of the Amber House experience, and the Resident Care Coordinator is an important part of this growing team. The coordinator will provide personal and restorative nursing care, and assist with ADLs, in accordance with approved policies/procedures.

Responsibilities

Day-to-day responsibilities include:

  • Safely administers quality skilled care in compliance with nursing policies and procedures, physician orders, and to provide external regulatory requirements for established plan of care.
  • Documents all aspects of residents care accurately, concisely, and promptly
  • Administers medication and treatments accurately, observing resident response, as evidenced by documentation in the medical record and lack of negative outcomes.
  • Maintains and coordinates communication among resident/significant other, staff, and physician as evidenced within the clinical medical record, client, and health care team feedback.-Effectively communicate residents changing needs and plan of care among staff members.-Attends physician rounds on assigned residents and communicates observations regarding resident’s condition. 
  • Observes the activities of all nurse aides assigned to area of responsibility. Guides, teaches and develops nurse aides to ensure safe, quality care for geriatric residents. Monitors nurse aide’s activity and reports non-compliant acts to Director immediately for correction.
  • Maintains optimum nursing practice through participation in internal and external training enrichment programs geared toward geriatric residents.
  • Participates in staff development programs
  • Assists in the orientation/training of new nursing personnel.
  • Demonstrates sound judgments by taking appropriate actions regarding questionable findings or concerns.
  • Consistently works in a positive and cooperative manner and assists other nursing personnel
  • Maintains confidentiality of resident information at all times.
  • Complies with the facility’s infection control policies and procedures and the Exposure Control plan that reflects OSHA Bloodborne Pathogen Standards to ensure a safe working environment for self and others.
  • Anticipates and takes action to maintain a safe environment.
  • Incorporates safety/risk management standards into daily work practices. Uses all safety equipment provided appropriately.
  • Performs all other duties as assigned by Director.

Qualifications

Because of the nature of the Amber House environment as well as its residents, the successful candidate will display strong compassion, interpersonal and communication skills, and a genuine desire to assist an aging/frail population. Applicants must have a current Pennsylvania LPN. A minimum of  one-year experience in long term care or other clinical nursing experience is preferred.  Marketing experience is preferred.
