At the center of this mission is the nursing staff, including Registered Nurses who provide direct and indirect care to assigned members, both in-home and in-center. This position requires a motivated self-starter who can work effectively and independently, as well as in partnership with medical and non-medical personnel, to deliver the most comprehensive care available. As such, nurse candidates must have strong clinical competency and advanced knowledge/skills.

Senior LIFE is an innovative program that uses a team approach to provide seniors with medical, social, nutritional and rehabilitation services. These services are provided at the Senior LIFE  Center and are accompanied with in-home nursing care, personal care, and other homecare services as needed. Senior LIFE makes it possible for seniors to remain at home, enjoying the comfort and reassurance of familiar surroundings while receiving the care they need.

 

Senior LIFE is a Medicare Program of All-Inclusive Care for the Elderly (PACE) operating as a LIFE (Living Independence for the Elderly) program in the state of Pennsylvania.

Responsibilities

Responsibilities including:

 

  • Identifying medical needs for individual members, and setting care priorities accordingly
  • Developing ongoing Plans of Care, in partnership with Senior LIFE interdisciplinary teams
  • Completing initial and periodic assessments for clinic- and home-based members, to develop and revise care plans according to emergent and pre-existing needs
  • Interpreting, recording and acting on diagnostic results, and performing advanced nursing techniques and critical care procedures
  • Identifying and assessing major medical problems, and communicating potential needs to a physician or specialist
  • Coordinating on-call rotations and participate, as needed, including triage and management of general member medical needs outside of standard center hours
  • Recording member care delivery, including all variations from established plans with rationale for those changes
  • Documenting all pertinent information related to member encounters in the EMR, at point of service or immediately following
  • Completing on-call documentation, including logs and corresponding medical documentation and inputting critical information into Data Pace
  • Collecting member data and completing all required forms according to center, state and federal policies
  • Assessing monthly MAR and comparing status to current physicians’ orders and medication lists, then updating and evaluating, as needed
  • Identifying development areas, including individual member enhancements and overarching LIFE Center opportunities for growth and expansion, then developing and implementing these improvements
  • Implement ongoing assessment strategies to best evaluate and enhance care plans for individual members and the center as a whole
  • Educating family and caregivers on advanced care plans and in-home needs
  • Managing caseload up to 25 members

Qualifications

Because of the nature of the Senior LIFE community, all applicants must show a deep understanding of age-related characteristics and unique needs associated with eldercare. While this is a highly independent role, it equally requires that candidates thrive in team-oriented environments and be able to think outside of the box to ensure the most innovative, comprehensive care for members. Applicants should have at least one year of experience working specifically with an elderly or aging population, and hold a current RN license. Managerial experience is preferred, but not required.
